use of the trephine for creating an opening in the skull or in the sclera.
dental trephination surgical creation of a fistula by puncturing the soft tissue and bone overlying the root apex to provide drainage. Called also apicostomy.

I cluck like a chicken, howl at the moon, and drill a daily burr hole into my skull to allow all my knowledge of ventilators to seek asylum elsewhere.
But we would like to drill some burr holes in his head.
The procedure involved a surgical opening of the skull, not a burr hole, and tumor resection, not a biopsy.
