adverse drug reaction,
n a detrimental outcome from a drug. Two types of ADRs exist: Type 1 results from dosage mismatch and Type 2 from rare conditions often as a consequence of a small dose. See also risk or sensitive type.

This can be compared with data from the Australian Incident Monitoring System showing that most medication incidents occurring in hospital were categorised as omissions (>25%), overdoses (20%), wrong medicines (10%), drug of addiction discrepancy (<5%), incorrect labelling (<5%) or an adverse drug reaction (<5%).
Some researchers (24,25) also have cited fatigue as an adverse drug reaction.
Supplemental injury and external cause codes assigned during cause-of-death coding differentiate radiation and toxic gastroenteritis, as well as gastroenteritis attributable to adverse drug reactions (19).
