The ghrelin hormone is unique as it gets activated via acylation (the addition of a fatty acid) by a specific enzyme (ghrelin O-acyl transferase, or GOAT).
The modification of the active group in the drug through acylation, phosphorylation, glycosylation, nucleotidylation or ribosylation by the organism could make the former innocuous.
