Formally called Haglund deformity. An exostosis over the posterolateral aspect of the calcaneus caused by recurrent friction, ± bursitis
Cause Multifactorial—e.g., abnormal mechanics leading to excess motion of calcaneus, congenital deformity of calcaneus, irritation from women’s shoes known as ‘pumps’
Management Open-heeled/flat shoes, moleskin padding of calcaneus, cryomassage, orthotics to control excess motion; surgical excision

; Achilles paratendinitis Achilles tendon (tendo Achilles: TA) overuse syndrome; presenting in acute (<2 weeks' duration), subacute (3-6 weeks' duration) or chronic (>6 weeks' duration) forms; treated by RICE(P) and immobilization; postacute-phase remobilization includes a heel raise (to reduce tendon traction), biomechanical assessment and orthoses as necessary to control identified foot/limb anomalies (e.g. true or functional rearfoot varus, forefoot varus, forefoot valgus, plantarflexed first ray, limb-length discrepancy); it predisposes to total or partial tendon rupture (see Achilles tendon rupture)
  • acute Achilles tendinitis localized (sometimes nodular) TA swelling and rupture of tendon fibres, characterized by pain on activity, relieved by rest

  • chronic Achilles tendinitis TA pain in the morning on first standing, or after activity (e.g. active ankle plantarflexion; going on to tiptoes), with local tenderness, inflammation and a fusiform TA swelling 5 cm superior to its calcaneal insertion

  • subacute Achilles tendinitis diffuse swelling along the length of the tendon with pain and palpable crepitus on foot movement/posterior muscle group contraction
