(1) Cardiology Alternans before Cardioverter-Defibrillator
Endocrinology. A clinical study of noninvasive stratification of patients with ventricular arrhythmias to determine the need for ICD by undergoing prior standard electrophysiologic and T-wave alternans testing
Results Nisoldipine or enalapril as initial antihypertensives appeared to stabilise renal function in hypertensive type 2 diabetic patients without overt albuminuria over 5 years; stepped up control decreased all-cause mortality
Method Candidates patients with past prior MI, decreased ejection fraction, and non-sustained ventricular tachycardia on routine screening using a Holter monitor; such patients have shown better survival with ICD therapy if they demonstrate inducible VT during an electrophysiology study; the ABCD trial was intended to determine whether people with normal EPS results but abnormal TWA findings would also benefit from ICD placement
